Membership Pricing

Monthly Fee: $250 (banked into your Boulevard wallet for future treatments)

One-Time Enrollment Fee: $250 (includes Welcome Bag)

• Total Due at Signup: $500

Membership term begins on the 1st of the month following initial enrollment.

Freezing a Membership

Need to pause for a bit? No problem.

• Memberships are eligible to be frozen after 2 payments.
• Memberships can be frozen once during a 12-month period for 30 to 90 days.
• No complimentary treatments or benefits can be used during the freeze period.
• During the freeze period, any accrued funds may be used at the current non-member prices.
• Your term will automatically extend by the length of the freeze.

Cancellation Policy

• Membership requires a 12-month commitment.
• Early cancellation is allowed only if relocating more than 25 miles away (with documentation).
• A 30-day written notice is required to cancel.
• Membership payments are non-refundable once processed.

Renewal Policy

• Per New York State regulations, notification of membership renewal will be sent via the email on file in Boulevard 15-45 days prior to the renewal date.
• Membership will auto-renew annually unless canceled with 15 days' notice before the renewal date.
